My mother and I received an upgrade from the lowest level to a Penthouse on a 4-day Voyager cruise two years ago. Though I never quite saw the point of the butler, Mom sure got a kick out of having one. I don't recall being asked to pay extra for the upgrade, though it's possible they asked for some small amount.
